Requests for medical records can be made by submitting a HIPPA Release of Information form to the medical records department. You can also reach the medical records department by phone by calling 754-300-3120.
Forms can be submitted by emailing [email protected] or by faxing 888-887-3650. Please be sure to complete the form in it’s entirety and include a copy of the patient’s photo for verification purposes.
